California Supervisor and Operator Permit
- Apply for permit
- Register to take Exam
- Take the California Department of Public Health – Radiologic Health Branch test
Apply for Permit
- Submit California Licentiate Supervisor and Operator Permit Application
- Select “Fluoroscopy Supervisor and Operator Permit”
- Need a copy of California Physician license
- Takes about 30 days
- Cost $127.00
- Applications are paper only

Register for Exam
- Application is forwarded by California Department of Health to the American Registry of Radiologic Technologists (ARRT)
- Register for exam www.staterhc.org
- $175
Schedule Exam
- ARRT sends a candidate status report which provides information to schedule exam appointment with Pearson VUE test center
- Schedule exam
- Exam must be scheduled and taken in a 90 day exam window defined in candidate status report

Results
- Results are mailed to location indicated on application
- Results take about 6 weeks
- Score report indicates scores on each domain
- Minimum scaled score required to pass 75
